HEALTH PROGRAMS

DIABETES
Diabetes screening and education in partnership with Community Health Centres and other like minded agencies.

Mission
Aim of LHFC is to provide services and education to improve the quality of life for those exposed and living with Type 2 diabetes

Goals
Educate and provide support for those at risk or living with Type 2 diabetes.
Build partnership with other agencies who share the same vision of better health for marginalized communities. Direct those affected by diabetes to resources and groups to improve their lives.

SICKLE CELL
Sickle Cell screening in partnership with Community Health Centre and other agencies

Mission
Offer education and support for those living with Sickle Cell.

Goals
Sensitize the community about sickle cell and support resources available
